Edinburgh's first medical officer of health, Dr HD Littlejohn, gave the answer in one of the finest health reports ever written on a British city.

Paul Laxton and Richard Rodger use maps and case studies to illustrate new research on Littlejohn's diagnosis, presecription and the vested interest he faced.
